As we celebrate Memorial Day, we take a moment out of our lives to remember and honor those who serve our country. These people have dedicated their time and lives to ensuring our freedom. Some lose their lives or spend many months away from their families. Many others come back from service forever changed because of injury or substance abuse and mental health disorders. We should be doing all we can to help our service men and women recover from these serious issues.

Lasting Effects of War

Most of us know the dangers soldiers face when they enter the military. Not only do they dedicate time serving away from their families, but they put their lives at risk. For those that make it home from combat safely, there is the increased risk for mental health disorders and substance abuse. Brain injuries and traumatic experiences are most often to blame. Post Traumatic Stress Disorder (PTSD) is a serious disorder that stems from experiencing a distressing event such as war. It causes the person to be plagued with anxiety and flashbacks that can last years.

Our soldiers also return from service with possible physical injuries and with lasting mental disorders, and then they are expected to jump back into society. Transitioning back to living at home and being a father, mother, sister, or brother again can be difficult. Many service men and women struggle with it.

Self-Medicating

The solution many military personnel turn to is substance abuse. It is a way for them to deal with the pain and uncertainty that they feel. Many soldiers binge drink because they don’t know how to live their life again. Others do drugs to forget the things they’ve been through. The result is that our soldiers are more likely to abuse drugs and alcohol than the general public. A statistic coming out of Arizona last week helped draw attention to the problem. It said that drug and alcohol abuse is four times more common in veterans coming back from Iraq and Afghanistan than the general public and veterans from other wars.(1) Many veterans feel there is no other way to get relief than to self-medicate with drugs or alcohol.

Help for Veterans

We might think that these people just need to get treatment and therapy for all the things that are bothering them. But many do not seek help when they should because of embarrassment or denial. Others that do want help are stuck trying to wade through red tape and restrictions on benefits through the military.

This Memorial Day, as we remember our service men and women, let’s not assume their struggle is over just because they are home. Encourage anyone you know who needs help to find it. SAMHSA has a web page of links that can help military personnel and their families with substance abuse and mental health problems.

Even though drug abuse is common among athletes, there are some sports that are less prone to drug abuse among participants. Racing is one of those sports, thankfully. That does not mean drug abuse never finds its way in; it simply means that when drug abuse is found, it is taken care of pretty quickly.

Zero-Tolerance

Given the nature of racing and the danger of the sport, you won’t find many competitors who would risk their safety by doing drugs. Most people feel there is little need for performance enhancing drugs in racing because the power is in the car, not the driver. Pit crew teams and drivers often have a closeness that helps keep people from sneaking drugs. And since crews need to trust each other to ensure everyone’s safety, coming to work high would mean putting a lot of people’s lives in jeopardy.

All this does not mean that drug abuse never occurs in racing. It does. A crew member from Richard Childress Racing’s No. 31 team, Gary Frost, has been suspended for violating NASCAR’s substance abuse policy. According to a press release, substance abuse is taken very seriously. "Richard Childress has zero tolerance when an employee fails a NASCAR drug test due to an illegal substance,"  Richard Childress said in a statement. "As a result, that person's employment is terminated. It is important for our fans and our partners to know illegal drug use at RCR is simply not tolerated."

A Clean Sport

Gary Frost is certainly not the first person to be suspended from racing, but he is one of only a small number of people who have. Most people credit that fact to the swift action that is taken by NASCAR in cases like this, and the message that comes along with that action. If a driver or team member is found violating NASCAR’s drug policy, the person is usually banned from the sport. Unlike other sports where players can go through mandatory rehab, do their time, pay their fine, and then get back in, NASCAR drivers often face a complete career change if they are caught with drugs. “The responsibility here rests across the board — with the drivers and competitors, owners and teams and NASCAR,” NASCAR spokesman Kerry Tharp said. “A positive test results in severe consequences and is a career-changing moment for that person. No system is flawless; but we believe our zero tolerant policy that is in place has served the sport well.” (2)

For anyone trying to sneak through the cracks, NASCAR enacted random drug testing in 2009. Most drivers and teams have been supportive of the drug testing because they know how important it is to everyone’s safety that competitors remain sober. Most NASCAR participants agree that drugs are unacceptable in this sport, and that attitude has helped make the sport as clean as it is today.

Nitrates Make You Faster and Stronger

Drinking a half-liter of beetroot juice before a
time trial helped male bicycle racers ride an incredible 2.8
percent faster over both 4- and 16-kilometer courses
(Medicine & Science in Sports & Exercise, June 2011). This
is an extremely well-performed study because all of the
bicycle racers in the study rode faster after drinking a half-
liter of normal beetroot juice (6.2 mmol nitrate) than they
did after taking the same volume of beetroot juice that had
had the nitrates removed (0.0047 mmol nitrate). Furthermore,
the authors measured blood levels of nitrates and showed that
the nitrate-rich beet juice raised blood levels of nitrates
from 241 to 575 nm and reduced the oxygen cost of the
exercising muscles. The dose of nitrate used in the study
(6.2 mmol) is 4 to 12 times greater than what the average
person takes in each day. None of the subjects consumed
dietary supplements.
ANY FOOD SOURCE OF NITRATES, NOT JUST BEETROOT JUICE:
Beetroot juice is high in nitrates which have been shown in
many studies to reduce the amount of oxygen needed to fuel
muscles during exercise and to improve an athlete's ability
to tolerate high-intensity exercise. How fast you can race
on a bike is limited by the time it takes to bring oxygen
into exercising muscles to turn food into fuel for contracting
muscles. Since nitrates bring oxygen faster into muscles,
eating any food that is high in nitrates can help you ride
faster.
HOW NITRATES MAKE YOU FASTER: Inorganic nitrate is
converted to active nitrite and, subsequently, to nitric oxide
(Nat Med. 2003;9:1498-505). The most efficient form of energy
for exercising muscles comes from your mitochondria in muscles
which require less oxygen after a person takes nitrates
(Free Radic Biol Med. 2010;49:S109).
NITRATES IMPROVE ATHLETIC PERFORMANCE: Sodium nitrate
has been shown to reduce the oxygen cost of cycling (J Appl
Physiol. 2009;107:1144-55), increase power of knee extensor
exercise (J Appl Physiol. 2010;109:135-48), and help athletes
run faster (J Appl Physiol. 2011;110:591-600)
HOW MUCH NITRATE BEFORE RACING? In this study,
the racers were given liquids because liquids pass through their
stomachs and into their bloodstreams much faster than solid foods
do. You can get large amounts of nitrates from celery, cress,
lettuce, radishes, beets, spinach, Chinese cabbage, endive,
kohlrabi, leeks, parsley and many other vegetables (Am J Clin
Nutr. 2009 Jul;90(1):1-10). Almost all of the nitrates in
vegetables are absorbed into the bloodstream.
CAN YOU TAKE IN TOO MUCH NITRATES? Nitrates are safe
and healthful when eaten in vegetables and fruits. However,
nitrates can be converted to nitrites which can combine with amino
acids in foods to form nitrosamines which are potent
cancer-causing agents. Nitrates (with an A) in plants are
healthful. Too much nitrites (with an I) can be harmful.
AVOIDING NITRITES: Bacteria convert the healthful
nitrates to the harmful nitrites in vegetables. Fresh
vegetables are usually resistant to bacterial infections.
However, once a vegetable is cooked, bacteria reduce nitrates
to nitrites. Therefore, cook only the amount of a vegetable
that you plan to eat right away. Leftovers should be stored
for no more than a day or two in the refrigerator. Freezing
inactivates bacteria, so commercially frozen cooked vegetables
usually have low levels of nitrites. Pureed foods are much
higher in nitrites. Boiling vegetables reduces nitrate content
(Toxicol Lett. 2008(Oct 1);181(3):177-81).
SHOULD YOU TAKE NITRATE PILLS? Definitely not!
Excessive nitrates in grass and grains have killed ruminant
animals. Bacteria in the stomachs of ruminant animals
rapidly reduce nitrates (with an A) to nitrites (with an I).
Normally, the nitrites are converted to ammonia to be used by
bacteria as a source of nitrogen, but nitrites can convert
hemoglobin in blood to methemoglobin, which cannot carry
oxygen and the animal can smother to death.
In humans, excessive nitrites can accumulate also
and prevent oxygen from attaching to red blood cells. This
is called methemoglobinemia which can prevent you from even
being able to move, and has killed some infants and adults.
This condition can be caused by drinking very large amounts
of stale juice from cooked spinach or other vegetables high
in nitrates. On the other hand, you should not worry about
nitrates in fresh vegetables harming you.
SHOULD YOU EAT VEGETABLES CONTAINING NITRATES ALL
THE TIME? So far, nobody has proven that a high vegetable
diet will make you a better athlete. It was relatively easy to
set up a study in which a single dose of nitrates is given
before a time trial. However, it takes far more planning and
work to set up studies in which athletes are on long-term,
high-vegetable diets and compare them to other athletes on
low-vegetable diets. We have to await further research to
prove that a high-plant diet will make you a better athlete.
I think it will.
NITRATES HELP TO PREVENT HEART ATTACKS: I recommend
that you eat a high-vegetable diet anyway because it can prolong
your life and help to prevent certain cancers, heart attacks and
many other diseases. The Mediterranean Diet and the DASH Diet
have both been shown to lower cholesterol and blood pressure,
and help to prevent heart attacks. These diets are high in
fruits and vegetables that are rich sources of nitrates.
Many studies show that nitrates in vegetables lower high
blood pressure. (Hypertension. 2010;56:274-81). They do this
by widening blood vessels to increase oxygen availability to
muscles. Nitrates also help prevent heart attacks during
intense exercise (Cardiovasc Res. 2011(Feb 15);89(3):499-506).

Dear Dr. Mirkin: Should I take antioxidant pills as part of
my training program?

No; the current issue of Journal of the American College
of Sports Medicine shows that antioxidant pills prevent the
major mitochondrial benefits of athletic endurance training
(Medicine & Science in Sports & Exercise, June 1, 2011).
Rats that trained on a treadmill increased the enzymes that are
necessary to increase the number and size of mitochondria.
Rats who trained on a treadmill and were given two powerful
antioxidants, vitamin E and alpha lipoic acid, did not increase
the enzymes that are necessary to increase the number and size
of mitochondria.
HOW HUMANS GET THEIR ENERGY FOR EXERCISE: Humans convert
food to energy most effectively in the mitochondria, hundreds
of small chambers inside muscle fibers. They need oxygen to
do this. The limiting factor to how fast and long you can
move is the time it takes to move oxygen into muscle fibers.
Athletic training makes you faster and gives you greater
endurance by enlarging and increasing the number of
mitochondria in muscles.
ANTIOXIDANT PILLS CAN HARM: People who take 1000 mg/day
of vitamin C and 400 IU/day of vitamin E do not gain the
benefits of increased insulin sensitivity when they exercise
(Proceedings of the National Academy of Sciences, May 12,
2009).
- When blood sugar levels rise too high, sugar sticks on
the surface of cell membranes and can never get off. The
attached sugar is converted in a series of chemical reactions
to sorbitol that destroys cells.
- Contracting muscles help to prevent this damage by
removing sugar so fast from the bloodstream that blood sugar
levels do not rise too high.
- Food is converted to energy to power your muscles by a
series of chemical reactions that shuffle electrons from
molecule to molecule.
- This occurs primarily in the mitochondria, small
energy-producing chambers in cells, that number anywhere from
a few to thousands in each cell.
- As electrons are shuffled to produce energy, extra
electrons can accumulate. They can either end on hydrogen
atoms to form water and become harmless, or they can end up on
oxygen atoms to form free radicals that can damage cells. This
can cause cancers, heart attacks and other life-shortening
conditions.
- Exercise speeds up the reactions that turn food into
energy, so exercise increases the production of free radicals.
- The body responds to this increased production of free
radicals during exercise by producing tremendous numbers of
antioxidants that sop up the free radicals and render them
harmless.
- Exercise prolongs life and prevents heart attacks and
cancers by causing the body to dispose of free radicals by the
increased production of antioxidants.
LET THE BUYER BEWARE: If you exercise and take
antioxidant vitamins C and E, you prevent your own body from
making large amounts of antioxidants during exercise, so more
free radicals (oxidants) accumulate in your body and more
cells are damaged.
Many studies show that:
- Taking large doses of beta carotene (pro-vitamin A)
increases risk for heart attacks in men and increase risk for
lung cancer in smokers.
- Taking large doses of vitamin C does not prevent colon
cancer, and does not prolong life in people with cancer.
- Taking large doses of vitamin E or selenium does not
prevent lung cancer, heart disease or stroke.
MY RECOMMENDATIONS: Exercise every day, and get the
antioxidant vitamins and other nutrients your body needs from
foods, not from pills. Eat a wide variety of foods including
large amounts of fruits, vegetables, whole grains, beams, nuts
and other seeds. If you want to take Recommended Dietary
Allowances of vitamins in pills, go ahead; there is little
evidence that you will harm yourself. However, when you take
large doses of any vitamin, you don't have the foggiest idea
whether you are harming or helping yourself. I do not
recommend large doses of vitamins to anyone.

Saturday if I have no alcohol the rest of the week?

Yes! That is called binge drinking. Taking more than
2 drinks a day on a regular basis will shorten your life and
increase risk for certain cancers, heart attacks and strokes.
A drink is five ounces of wine, 12 ounces of beer and 2/3rds
of shot glass. Taking more than two drinks a day, just once
a week, can impair your memory. Binge drinking students
perform worse on memory tests, both on immediate and delayed
recall (Alcoholism: Clinical and Experimental Research,
published online May 16, 2011).

Amidst a surge of new and trendy drugs is one that is more dangerous than the rest. This drug is more addictive, more lethal, and cheaper than many other drugs that people are familiar with or that are new on the scene.

Made with Toxic Chemicals

Oxidado, or “Oxi” for short, is taking lives and ruining others, yet it remains popular among certain groups of people. Oxi is derived from cocaine and looks like crack, but it also contains chemicals such as gasoline, battery fluid, and acetone. Oxi is made in people’s homes, using cocaine paste that has been soaked in gasoline. Other household solvents are added during the process, making this drug more toxic than most others. The director of a drug research center in Rio de Janeiro, Ivone Ponczek, said that Oxi was more dangerous than crack because it was easier to be produced. "While crack needs a mixture of sodium bicarbonate and others, and still needs a little know-how to be done, Oxi can be handmade, can be made by anyone," (1) Unfortunately, this makes the drug very plentiful in places like Brazil.

An Unfortunate Brazilian Export

Oxidado, which means “rust”, originated in Brazil, possibly as early as the 1980s, but has increased in popularity since 2008. It is still a drug used primarily in Brazil, but word of it is spreading to other countries as well. It won’t be long until Oxi, which is causing a public health emergency in Brazil, will be causing alarm throughout the world. Because of how it is made, Oxi is up to five times cheaper than crack cocaine. The average price for one rock of Oxi is $1 to $3, while cocaine is often found for $60 a rock.

Dangers of Oxi

Before anyone goes looking for this drug to find themselves a cheap high, they should heed the warnings. Most people who use Oxi even one time get hooked. Most users die within one year. According to one researcher, Alvaro Mendes, who has studied the drug and its effects, “In the 15 years I have been working with chemical dependency, I have never seen a drug with such a potential of destruction as Oxi.” (2) Mendes calls Oxi the most addictive and powerful drug he has ever seen. Out of the 80 Oxi users he studied, 34 died during the study.

Oxi has the potential to grip its users so powerfully that they can think of nothing other than their next hit. The effect of smoking Oxi lasts only 3 to 5 minutes, but the damage to the body can be irreversible. Oxi causes users to quickly develop liver problems, severe stomach aches, vomiting, and diarrhea, and users lose weight quickly. But instead of being able to stop taking the drug or seeking help, users stop eating and are compelled to spend their time in abandoned buildings getting high. Most addicts forget about not only their responsibilities and families, but also personal hygiene and basic human needs. As one addict said, “You lose your shame when you take Oxi.” (2)

A Quick Cure for Fatty Liver

More than one third of North Americans will suffer
from a fatty liver that can progress to liver inflammation,
cirrhosis and liver cancer. Storing excess fat in your liver
markedly increases your risk for diabetes, heart attacks,
strokes, obesity, certain cancers and premature death.
Restricting carbohydrates is more effective than cutting
calories or limiting fat for individuals who want to reduce
the amount of fat in their livers, according to a study in
the American Journal of Clinical Nutrition (April, 2011).
Eighteen participants with fatty livers ate either a
low-carbohydrate or a low-calorie diet for 14 days. After
two weeks, MRIs showed that those on the low-carbohydrate
diet lost more liver fat.

HOW FATTY LIVER CAUSES DISEASE: Having a fatty liver
*blocks insulin receptors to prevent your cells from responding to
insulin which causes *a high rise in blood sugar which causes
*your pancreas to release huge amounts of insulin which
*converts sugar to more triglycerides which *fills up your
liver with fat to cause Non-Alcoholic Fatty Liver. As this
process continues, you develop *excess fat in your body
(obesity), *high blood sugar (diabetes), *high insulin and
triglycerides levels (which cause heart attacks), and
*inflammation (that causes certain cancers and many other
diseases).

HOW YOUR BODY USES CARBOHYDRATES: Carbohydrates are sugars in
singles, doubles, thousands and millions. Before any
carbohydrate can get into your bloodstream, it must first be
broken down into a single sugar. Of all sugars, only four
(glucose, fructose, galactose and mannose) can pass from your
intestines into your bloodstream leading only to your liver.
Of the four sugars, only one, glucose, can pass from the
liver into the general blood circulation. Galactose and
mannose are quickly broken down in your liver and do not
reach your general circulation. Fructose is converted to
glycogen, the storage form of sugar in your liver. However,
the liver can store only so much glycogen. As soon as all
glycogen stores are full, most of the fructose is then
converted to a type of fat called triglycerides. If large
amounts of triglycerides accumulate in your liver, you
develop a fatty liver.

HOW CAN YOU TELL THAT YOU ARE TAKING IN TOO MUCH FRUCTOSE?
*You start to store most of your fat in your belly.
*Your blood triglycerides, insulin and sugar start to rise.
*Your good HDL cholesterol goes down.

HOW CAN YOU TELL THAT YOU HAVE A FATTY LIVER?
Your triglycerides will be higher than 150, your HBA1C (sugar
stuck on cells) will be above 5.7, your C-peptide (which
measures insulin your body produces) will be higher than 1.9
ng/mL or .62 nmol/L, your HDL cholesterol will be below 40,
and a sonogram of your liver will show the fat destroying
your liver. Your doctor now should tell you that you have
metabolic syndrome and are headed for a premature death. If
you don't like this prediction, you must:

*Avoid all sugared drinks (except during prolonged and
intense exercise). Fruit juice and table sugar are just as
damaging as the high-fructose corn syrup (HFCS) found in most
commercially sugared drinks.
*Markedly restrict all foods made from flour (bakery products
and pastas).
*Eat plenty of vegetables, berries, beans, nuts, and fish.
Avoid meat, particularly cured meats or sausage (European
Journal of Clinical Nutrition. March, 2011).
*Lose weight
*Exercise (first check with your doctor because having a
fatty liver markedly increases your chances of suffering a
heart attack)
*Get your vitamin D3 level above 75 nmol/L (Diabetes Care,
published online April 22, 2011)

Dear Dr. Mirkin: What is the latest research linking red
meat to cancer?

Eating red meat is associated with at least 17
different cancers and very strongly linked to colon cancer.
Ajit Varki, MD of the University of California, San Diego,
has found a sugar-protein, called Neu5Gc, found in all
mammals except humans. He has shown that humans who eat
mammal meat make antibodies against this sugar-protein in the
same way that they make antibodies to invading germs, and
have their immunities turned on to cause inflammation.
Constantly producing antibodies to Neu5Gc can lead to chronic
inflammation, which can cause cancerous cells to develop and
grow. He has also shown that Neu5Gc is found in tumors in
meat-eating humans, and that the highest levels are found in
metastasizing tumors (tumors that spread throughout the body
and cause death).
Now Dr. Varki and his associates show that blood
tests for human antibodies to Neu5Gc can be used to predict
certain cancers, and that giving antibodies to Neu5Gc in very
high concentrations suppresses tumor growth (Cancer Research.
May 1, 2011). The researchers found that high doses of
purified human anti-Neu5Gc antibodies can kill tumors
containing Neu5Gc in mice and humans. These findings point
to a dual response of anti-Neu5Gc antibodies that can either
stimulate tumor growth at a low dose or suppress tumor growth
at a high dose. This means that low doses of Neu5Gc
antibodies stimulate cancer cells to grow, while high doses
attack and kill the same cancer cells.
If other researchers confirm Dr. Varki's findings,
he is a likely candidate for a Nobel Prize in Medicine.

Dear Dr. Mirkin: What should long distance runners and
bicycle racers eat to help them recover faster?

During their recovery periods, they should eat huge
amounts of foods loaded with protein and carbohydrates. Since
the 1930s, athletes have known that they can recover faster
from hard workouts by eating extra carbohydrates: fruits,
vegetables, and whole grain bakery products and pastas. The
carbohydrates refill their muscles with glycogen (stored
sugar). Sugar requires less oxygen to fuel muscles during
exercise than fat or protein. Anything that reduces oxygen
requirements during exercise allows the athlete to move
faster and stronger, and have greater endurance.
Recent research from the University of Birmingham in
the UK shows that athletes can recover even faster if they
also take in extra protein (Medicine & Science in Sports &
Exercise, April 2011). This is the first study to show that
several days of high-protein feeding hastens recovery during
several days of high-intensity training. It doesn't make
any difference if they get their extra protein from dairy
products, meat, fish, chicken, beans, seeds or nuts. The
study showed that trained male cyclists could ride
significantly faster time trials on their very intense days
when they were given a diet with extra protein. The high-
protein diet contained 3 g protein/kg/day) and the normal
diet contained 1.5 g protein/kg/day.
All athletic training is done by stressing and
recovering. The athlete takes one or more intense workouts,
and then when he feels very sore, cuts back on the intensity
of his workouts for a while. Anything that will help him
recover faster will allow him to take his next series of
intense workouts sooner and he will become stronger, faster,
and have greater endurance. Previous research has shown that
taking extra protein soon after finishing an intense workout
can hasten recovery (Am J Physiol Endocrinol Metab. 2004;287
(4):E712-20). This is explained by the fact that a high
carbohydrate load immediately after finishing the intense
workout causes the pancreas to release large amounts of
insulin which drive the extra protein (amino acids) into
muscle cells so they heal faster (J Nutr. 2000;130(10):2508-13).
